The paperwork I see received after my visit with the doctor was blank. Had a doctor's signature, but no symptoms are reasons why I needed the card.

I have reviewed your paperwork and everything appears to be in order. The physicians signature is there at the bottom of the document in the Physician Information section.
The doctor did not specify the medical condition because it is an optional section and most patient do not want their medical conditions listed for privacy reasons.
The first physician signature halfway down the page (Certification of Necessity of a Caregiver section) is blank because that is only signed if a caregiver is required.
If you have any questions or require additional assistance please reply to the email that had the medical cannabis certificate attached or call our office at 804-893-5068. If something is incorrect or you needed a caregiver please let us know.
