My team and I have a great experience using forms.app. As a small non-profit we find this app very professional looking especially when we do surveys with externals. Their Ai function also helps us analyse our data collected which is very useful for us to better improve our services to our partners.

As a small grocery shop, it was easy to create our on-line order services, using the proposed format. It has all functions that we need for now.

After researching the best way to accept submissions for a photo contest, I found Forms.app. This platform is so full featured that even in the "forever free" plan, I could make a detailed entry form and accept uploaded images. I can even star my favourites as they come in for easy reference.
Checking the responses on the app is as easy as the desktop.
I love how everything is able to be exported in all sorts of formats.
None of the "big names" had all of these features

I don't leave reviews typically but in this instance I feel i must. I've used many form/quiz type apps (eg. jotform, quizitri, etc.) but this platform and company stand out. First of all the prices are reasonable, not overpriced like most. For all the features you get (eg. weighting answers for point calculations, custom subdomain, etc.), this platforms beats the others. Are there occasional hiccups with the app, yes, just like all of em. But the support is awesome. They reliably respond to my questions and needs quickly--always same day or within a day or two. Yulia especially is great. They go above and beyond. Not sure what some of the negative reviews mean. I've had nothing but great service from them. I just renewed for another year and plan on telling others to use them, too.

Stay away from these unprofessional people you spend hours building a form then the disable the form and lo k on to your website you have to delete that page to get rid. Avoid like the plaige obsolute rubbish give a wide berth

I've used several online forms creators and have been disappointed in one way or another with all of them. I came upon forms.app and checked here for reviews. I thought I had found the Holy Grail of forms apps. I was wrong.
Granted, the UI is clean and easy to use. And the upgrade pricing is reasonable. That's the end of the good news.
When things don't go as expected, you will get no help from the site. Every time I have searched the help pages, I've gotten zero results – and I'm no stranger to online searching. I know how to structure a query. Forms.app's help search utility is simply useless. The how-to videos are cute but provide only the barest minimum of instruction. When you've exhausted the online help facilities and need to get in touch with a human, then you find out what a poor choice you've made. The online chat team works only during business hours in Türkiye. If you're in another time zone, you're out of luck. In the central U.S., you'd have to contact them between midnight and 6:00AM. I am asleep in bed during those hours.
Edit: I requested a refund for the year's service I paid for in advance, and it was promptly issued. For that I added a star. But since their representative keeps bothering me about my review, I'm subtracting that star. I stand behind my original review. If you're in Türkiye, Ukraine, Scandinavia or eastern Africa, this platform will work for you. Elsewhere, not so much.

The website is really great. You can create such detailed surveys and have so many options in terms of both design and evaluation
