The bad to worst experience
On December 16, I received an email from Ms. Brown requesting that I complete a signature form to authorize the release of my medical records from Johns Hopkins.
On December 17, I called Johns Hopkins and spoke with Tracy, but was unable to obtain any meaningful information.
On December 18, I called again and spoke with Andrea, who explained the situation and outlined the next steps.
I called a third time on December 19 and spoke with Karen, who confirmed Andrea’s information and stated that my records are now being processed.
According to the Johns Hopkins Records Department (per Andrea), the initial request from Parameds was received on December 7. Johns Hopkins receives many requests from Parameds and consistently follows the same procedure: they do not accept digital signatures and have never done so. Andrea stated she was surprised that the request for my records arrived with a digital signature, and therefore, the request was returned.
Nine days after that initial request, on December 16, Ms. Brown sent me a manual form. I printed, signed, and returned it on December 17. Johns Hopkins received that second request, with a physical signature, on December 18.
I was informed that fulfillment will take 7–10 business days, placing completion around January 5 or later. In other words, the process of obtaining my records will take approximately one month solely due to preventable procedural errors regarding the signature submission. I asked Andrea if the request could be expedited; she said she would inquire. Karen confirmed today that the request is being expedited.
